I'd say it was most likely an engineering issue with a lack of oversight.
Engineers don’t over see means and methods of construction. It actually voids our liability insurance if we step into means and methods issues.

As I previously said, I don’t do bridges. The thing that’s odd to me and a coworker I talked to is that post tensioning puts extra compression into the concrete. The end diagonal of that truss is in compression. Why PT a member that is already in compression? You PT members in bending to counteract the tension. That member isn’t in tension.
