OK. I looked and it could be distilled into one word - "stupidity." Flying too close to the walls, not pulling the ripcord in time, fatigue from too many jumps in a day, etc. Also, it has Alpine weather too. Some 15-20K jumps are made a year, so that's a factor...The worst spot seems to be the Lauterbrunnen valley in Switzerland from fliers making "terrain mistakes." It's sometimes called the "Valley of Death" by that group. IDK why. I've been there a number of times and it's a standard glaciated valley, with vertical walls and numerous waterfalls (hence the name)...
