My stepson in Nashville has already had to have both eyes done for cataracts at age 50. The first one, he was in the surgery center, dilated, ready to start, when they informed him he was "out of network," and the simple surgery would cost thousands. They negotiated, when he started to leave...
I was scheduled for a series of injections to try and address some of my back issues. I kept trying to find out what the final cost would be, to no avail. Finally, they day before, the billing office called to let me know that it would be about $800 for each of the four procedures.

I canceled.

Reminds me of a recent incident. I've had a case of hives for months, without being able to track down the cause. I mentioned above, I think, that my dermatologist wanted to try me on Dupixent. I reminded her that it was a class of drug that someone with AGS shouldn't take. Once again, a "specialty" pharmacy raised its head. I think I mentioned in another thread that they wanted $2K per injection, two injections per month. I also declined...

For more than a decade, patients who've needed certain controlled medications have suffered from ill-advised, untenable policies the U.S. government has instituted, allegedly to mitigate the ever-surging numbers of drug overdose deaths. These policies have been a dismal failure on multiple fronts: Not only have deaths continued to surge, but the terrifying intrusion of the Drug Enforcement Administration (DEA) into the practice of medicine has had a chilling effect on patients and their physicians.

As the DEA relentlessly tightens production quotas on medications for pain and ADHD, it has begun tracking every pill, making doctors increasingly reluctant to prescribe any controlled drugs and leaving many patients in a lurch. Perhaps worse, DEA production quotas have caused the back-order of multiple drugs—an increasingly common burden for patients, even those fortunate to have doctors willing to risk a DEA drug bust for simply doing their job
By contrast, doctors in Australia, Canada, the United Kingdom, and throughout Europe have been using a fixed-dose, inhaled general anesthesia medicine that effectively reduces acute pain—a medication denied to Americans by a seemingly indifferent Food and Drug Administration (FDA).
This is a qualify of life issue, which to me, is more important than optics. It's certainly more important than kickbacks. :(

The FDA is slow as molasses in approving drugs. My go to anesthetic is Articaine. It was developed in France and available for years there and in other countries before being approved by the FDA. Articaine gives a more profound anesthesia than lidocaine and wears off in half the time.

I used to get one of my daily chemo meds, Xtandi, from Optum Specialty Rx. It’s now $3600 per month.

Crap like what these PBM’s are pulling is what necessitates government oversight. There’s no way market forces can correct how these guys are inflating the cost of medications.

I've noticed an amazing inconsistency about this. They either take an eternity or they approve with a puzzling speed. I'm reasonably certain there are a LOT of shenanigans going on with that agency, much of it centered around payoffs or job offers. I couldn't understand why Congress never noticed this enough to investigate until I figured out they were getting paid by the same lobbyists who are "encouraging" rapid approvals on certain medications.

I’m not quite as cynical. Though what you’re describing could be case, I prefer to think there is a system in place that is there to protect the public. If I’m right, the system needs to be streamlined.

Well sure, that was the whole idea and officially still is. But like any other entity, it is ripe for corruption. It's also worth pointing out that they are all fallible people and mistakes will be made.

I've outlined my problems with "specialty pharmacies." I wonder if they have any oversight at all...

I must say that so far, being a Medicare patient is the best situation I have ever found. I really wish there was some way to extrapolate this system to our general population, but that seems to be a dream. I am not looking forward to possible cataract surgery as the really good surgery is not covered by Medicare.
Preach! Medicare-for-all is available if we vote for it. My mother had her cataract surgery a few years back and had a wonderful experience. Hope you do, as well. You can always pay cash for better care, in any country.

I slipped going down the front steps 2 weeks ago. It had been raining and I let my guard down momentarily and there I went. No chance to brace myself so landed full speed into the front edge of a step behind me. Banged my head too!

Kinda shook it off and soldiered on. Mom had just passed away the day before and I had stuff to do. Figured it was just bruised ribs and they'd eventually get batter.

Finally reached my limit of pain endurance yesterday and went in to get checked out. Just got the results.....freaking broken rib!

Yeah, that's gonna be sore a few months. Hope it feels better soon, though.
